Elbow Pain When Training Arms: Why It Happens and How to Fix Your Technique
Epicondylitis, pain on the inner or outer elbow, is common among lifters who train arms hard. Learn the cause and how to adjust load and technique without stopping.
You swapped barbell curls for tricep pushdowns, but the elbow pain shows up anyway, whether on the inside or the outside of the joint. At first it's just a mild ache after arm day, the kind that fades within a day. A few weeks later, it hurts to turn a doorknob or lift a coffee cup. That's tendon damage, the injury known as epicondylitis, a common problem in strength training that gets mistaken for ordinary muscle soreness — which is why so many people quit training arms altogether, exactly when that tends to be the worst move.
Two different pains: tennis elbow and golfer's elbow
Despite the names, you don't need to play tennis or golf to develop these injuries — the names simply come from the movement first associated with each one in the medical literature. Lateral epicondylitis, known as tennis elbow, affects the wrist extensor tendons, which attach on the outer side of the elbow, just above the forearm. It tends to hurt when you extend your wrist against resistance, like gripping the bar tightly during a barbell curl or snapping your hand open to grab a weight off the floor.
Medial epicondylitis, or golfer's elbow, hits the flexor-pronator group instead, the tendons on the inner side of the elbow responsible for flexing the wrist and rotating the forearm. According to a review published on PubMed, it's actually a tendinosis — progressive tendon degeneration from repeated overload, not a sudden acute inflammation. It shows up most often in people who repeat wrist flexion and forearm pronation, such as during heavy biceps work or manual labor that demands a strong grip.
Why biceps and triceps training overloads the elbow
The elbow is a simple hinge, but it carries the combined load of two large muscle groups pulling in opposite directions: the flexors, like the biceps and brachialis, during curls, and the extensors, like the triceps, during pushdowns and close-grip bench press. When training volume climbs too fast — more sets, more weight, or more frequency than the tendon is ready to absorb — the tissue doesn't get time to adapt between sessions. The muscle around it recovers much faster than the tendon does, and that gap in recovery speed is exactly what creates the injury.

Two very common habits in the weight room make this worse: always using the tightest grip possible, which raises tension on the wrist extensors, and locking the elbow into full extension at the end of every rep, which shifts part of the load from the muscle onto the tendon itself. Neither habit is a problem on its own, in a single set. The trouble builds when they repeat workout after workout, week after week, with no variation in grip, angle, or range of motion.
A tight grip on barbell curls and a sharp, full lockout on tricep pushdowns concentrate force right at the point where the tendon is thinnest and least vascularized, near where it attaches to the bone. That's why the pain takes weeks to become noticeable — and also why it takes weeks to fade once you adjust your training.
The classic mistake on bench press and the tricep pushdown
On the bench press, elbow pain often shows up when the elbow is tucked too close to the torso on the way down — the tight tuck many powerlifters use to protect the shoulder. According to an analysis from Stronger by Science, that excessive tuck forces the shoulder into external rotation, which in turn demands more forearm pronation to keep a pronated grip on the bar. That extra pronation piles tension right onto the elbow tendons that are already irritated, turning a fix meant for the shoulder into a new problem at the elbow.
On the tricep pushdown, the equivalent mistake is letting the elbow drift away from the body during the extension, turning an isolation exercise into a disguised shoulder movement that stresses the elbow at an angle it wasn't built for. Keeping the elbow pinned to your side, with the cable moving in a straight line and without locking out hard at the bottom, removes that extra load without cutting triceps engagement.
How to adjust your training without stopping altogether
Stopping arm training completely is rarely the answer, even though it feels like the safest call on the first day of pain. The tendon adapts through controlled, progressive load, not through total rest — rest alone just postpones the problem until you go back to square one. Studies on eccentric exercises, where the muscle lengthens under load, such as slowly lowering a weight during wrist extension, showed improvements in pain, grip strength, and function in people with lateral epicondylitis.
The practical approach is to temporarily cut total load — less weight, fewer sets, or lower frequency — without eliminating the stimulus altogether. Swapping a tight grip for a neutral or semi-supinated one on some arm exercises takes some tension off the injured spot. Adding low-intensity isometric work, like holding the weight still for 20 to 30 seconds, before returning to full eccentric reps, tends to cut pain within a few weeks without wiping out the progress you've already built.
Cut your biceps and triceps volume in half for two or three weeks instead of dropping arm day entirely
Swap the tight grip for a neutral grip on curls and pushdowns while the elbow is sensitive
Avoid locking the elbow into full extension at the end of every rep
Add wrist isometrics, holding the weight still for a few seconds, before returning to the full movement
Rate your pain from 0 to 10 after every workout to track whether the adjustment is working
Keeping a training log during this adjustment period is what lets you see, week by week, whether the pain is easing as you cut back on load. Without that record, it's easy to fall into one of two traps: quitting for good out of fear, even when you don't need to, or coming back too soon and setting back a nearly-healed injury. That kind of daily tracking, the same habit that keeps a training streak alive, is what separates someone who recovers in a few weeks from someone who carries the same elbow pain for months.
Elbow pain rarely calls for total rest — it calls for the right dose of load, not zero load.
Strengthen your forearms to prevent a relapse
Anyone who's had epicondylitis is more likely to see the pain return if the forearm stays weak relative to the rest of arm training. Wrist extensors and flexors rarely get direct work in a typical routine: most workouts jump straight to biceps and triceps without ever specifically strengthening the muscles that stabilize the elbow during those movements. That imbalance leaves the tendon exposed every time training volume climbs again down the road.
Adding two direct wrist exercises — extension and flexion with light dumbbells, or a wrist roller with a weight tied to a rope — once or twice a week builds a base of load tolerance. This extra work doesn't need to be long or heavy: a few minutes at the end of arm day is enough to consistently lower the odds of the pain coming back.
Adjusting technique and load resolves most mild and moderate cases, but a few signs call for an evaluation from a physical therapist or orthopedist before you keep training on your own. These include pain that persists even at complete rest, a noticeable loss of strength for everyday grip tasks, visible swelling around the elbow, or pain that hasn't budged at all after four to six weeks of consistent adjustments.
Seeking help early in these cases keeps a mild tendinosis from turning into a chronic injury, one that takes far longer to resolve and often forces you to stop arm training completely for months — exactly the outcome the adjustments in this article are meant to prevent.
The goal is never to train through pain — it's to train with the right load for the tendon you have today, not the one you had before the injury.
